بتشوف الحرارة و اللون و الرطوبة و الملمس و ال edema و ال turgor و ال lesions

و ننزل باول ٥ خطوات ثابتين ف اي بروسيدجر بالنسبة للتقييم الصحي والاساسيات برضه
1) introduce your self ...
بتمسك الشيت اللي فيها حالة المريض و اسمه ثلاثي .. و تدخل عليه تقدم نفسك و تتأكد من هويته
Good morning! Mr. Yusif sameer mohamed, How are you doing? .. I'm Mr/Miss ..... I'm a nurse.

2) explain the procedure
بتبدأ تشرح اللي هتعمله 

say مثلا
I'm gonna to assess your skin color and texture.

3) prepare equipment
- strong, direct light
- pen light
- flexible centimeter ruler

4) hand washing, infection control procedure

5) Maintain body mechanics, privacy
بتدي للمريض الخصوصية 

 Gown و بغطيه بال Inpatient positionعن طريق انك تقفل الباب و الستارة لو هو في ال

6) expose the selected area for assessment and around الباقي with gown

بتكشف المنطقة اللي هتعملها فحص بس و تغطي الباقي

7) inspect skin color and uniformity ..
و خليك عارف ان ال ..
Color should be consistent with genetic background ..
و بنشوف ٣ الوان abnormal بتمثلوا ف التالي ..
* pallor .. شحوب
* Cyanosis .. زرقة
* Jaundice .. اصفرار

8) assess edema if present
بتفحص ال edema 

لو كانت موجودة طب بتعمل ايه بالضبط ؟ .. بتعمل 

pressing على  bony area في ال tibia و ال ankle

9) palpate & observe moiture and texture palmer surface of three middle finger
بتشوف رطوبة الجلد و الملمس و ده بيديك معلومات عن ال fever او ال metabolic activities

10) palpate skin temperture .. with back of hand
بتشوف الحرارة بضهر ايدك

11) Note skin mobility & turgor .. by lifting and pitching

clavicle بتسحب الجلد الموجود تحت ال

turgor و بسيبه لو مرجعش بسرعة يبقى دليل ع وجود 

fluid status و ده بيساعدنا ف تحديد ال 

12) inspect, palpate, describe skin lesions
و هنا بنشوف ال color .. ال odor .. ال size .. ال shape .. ال location .. ال elevation الارتفاع .. ال distribution  الانتشار ..
