 Nose assessment

1) introduce your self ..
و كالعادة بتدخل تقدم نفسك و تتأكد من هوية المريض من خلال ال sheet المكتوب فيه اسمه ثلاثي .. و تقول
Good morning Mr. Nour Mohamed Ahmed .. How are you doing? .. I'm Mr....

2) explain the procedure
بتشرح الاجراء ..
I'm gonna to assess your nose.

3) prepare equipment
بتحضر الادوات ألا و هي ..
• strong, direct light
• penlight
• gloves, mask
• otoscope with short, wide nasal tip or nasal speculum
• familiar odors

4) Hand washing ..
بتغسل ايدك و باقي ال infection control procedure .. زي مثلا ال mask و ال gloves

5) Maintain body mechanics .. provide privacy

6) inspect the external aspect of the nose
بتفحص الشكل الخارجي للانف بتلاحظ ٤ حاجات asymmetry ، و deformity , ال inflammation أو skin lesions

7) palpate the nose gently
بتضغط براحة على الانف .. و ده بيكشفلك عن وجود أي pain أو breaks أو masses

8) Test the patency of each nostril
بتشوف مدى نفاذية الهوا في كل فتحة عن طريق انك بتطلب من المريض يقفل ال mouth .. و بعدين occlude يسد يعني فتحة من الاتنين و يتنفس و يعيد التجربة مع الفتحة التانية

9) inspect the nasal cavity using either otoscope and penlight

10) Tilt the patient's head back, stabilize the instrument, and gently insert the speculum approximately 1 cm while avoiding the septum
بتميل راس المريض للخلف و بتثبت الاداة و تدخلها براحة تقريبا ١ سنتي و انت بتتجنب ال septum عشان ما يحصلش bleeding

11) Note the color and characteristics of the mucosa
بلاحظ لون و خصائص المخاط

12) Note the inferior and middle turbinates
و خليك عارف أن ال superior مش هتكون في مجال رؤيتك و مش هتعرف تلاحظها

13) Note any deviation of the nasal septum
بتشوف لو في أي انحراف في الحاجز بين الناحيتين و تاخد ف الاعتبار أن الانحراف ده شائع و عادي و ما يعتبرش مشكلة غير لو عامل انسداد لمجرى الهوا وقتها بس يبقى مشكلة

14) palpate the sinuses
- tenderness is elicited by palpation
لو حد عنده حساسية أو التهاب ف الجيوب الأنفية مجرد ما تعمله palpation بيحصل إثارة لل tenderness
- palpation of frontal and maxillary sinuses
و لازم تكون حذر و ما تحسش بألم

15) Test cranial nerve I:
Olfactory بالشم
بختبر حاسة الشم smell .. بتوجه المريض و تطلب يقفل عينه و occlude لفتحة من الانف و بتجيب حاجة ريحتها معروفة و تتأكد أن المريض ما اتعرفش عليها بالنظر و بسكدا
